Your Defiant Child by Russell A. Barkley
I'll have to get back to you with a real review once we've attempted to implement some of his strategies, but this seems like a useful book. I chose it because the author is a clinical expert in defiance and he claims that the strategies in the book are based on reams of research. We'll see.

I have a friend -- an admitted non-reader -- who turned to this book in desperation to deal with her oldest child. It's worked wonders for her and her child. (Her child's much less prone to explosive fits and tantrums than she was three years ago when we met her.)I have plans on getting around to reading it when my youngest, and most difficult, gets a bit older. :)
